To better understand your goals, we start with a complimentary phone call to gain insight into your condition. With this information, we can determine if you are an appropriate fit for physical therapy. We will also discuss a day and time to perform your initial evaluation.
Initiate
With the email you provided, all intake paperwork will be sent to you to be completed prior to your initial evaluation. This can be easily managed on your mobile device or computer through our client portal application.
Evaluate
The physical therapist will arrive at your home and provide a detailed assessment addressing all impairments previously mentioned. Based on the results of your tests and measurements, you will be provided with the first phase of your individualized home exercise program and discuss the recommended frequency for future visits.
